Important Information about Contacting CSEAP

CSEAP is currently able to accept your requests for appointments via the form below or by phone/voicemail. To schedule an appointment, please call 303-866-4314 or complete the form below. During this timeframe of mandated work-from-home, CSEAP is responding to voicemails and form submissions - we are unable to provide live response to all incoming phone calls. If you have call-blocking features activated on your phone, if your voicemail box is full, or if your voicemail box is not set up, we likely will not be able to reach you via return call or by leaving a message for you. In addition, we do not have the capability to use caller ID to return your call. If you are contacting us via phone, PLEASE ensure that you have a good network connection so that we can hear your message and return your call ASAP to set up an appointment.
